FIRST APPOINTMENT – WHAT DO YOU NEED

Here are some great tips regarding your appointment with Dr Sinnathamby.

New patients please:

  • Arrive 10 minutes early to fill patient details and medical history forms
  • Bring your referral letter from your GP or specialist
  • Any original X-rays, ultrasounds or MRI scans – Your imaging provider will be able to supply these
  • Medicare card
  • Private health insurance information (if applicable)
  • List of any medication/s or supplements that you are taking
  • Details of your other current medical providers (if applicable)

Current patients please:

  • Bring your private health insurance information (if applicable)
  • List of any medication/s or supplements that you are taking
  • Results from any further tests post your last appointment.

AT THE END OF YOUR APPOINTMENT

Depending on your treatment plan with Dr Sinnathamby, the next step may involve:

  • Booking a follow up appointment
  • Booking a surgery date and supplying you with the necessary information
  • Issue of medical certificate or certificate of capacity
  • Email or post letters regarding – Travel, insurance, GP, rehabilitation or external referrals.
