FIRST APPOINTMENT – WHAT DO YOU NEED
Here are some great tips regarding your appointment with Dr Sinnathamby.
New patients please:
- Arrive 10 minutes early to fill patient details and medical history forms
- Bring your referral letter from your GP or specialist
- Any original X-rays, ultrasounds or MRI scans – Your imaging provider will be able to supply these
- Medicare card
- Private health insurance information (if applicable)
- List of any medication/s or supplements that you are taking
- Details of your other current medical providers (if applicable)
Current patients please:
- Bring your private health insurance information (if applicable)
- List of any medication/s or supplements that you are taking
- Results from any further tests post your last appointment.
AT THE END OF YOUR APPOINTMENT
Depending on your treatment plan with Dr Sinnathamby, the next step may involve:
- Booking a follow up appointment
- Booking a surgery date and supplying you with the necessary information
- Issue of medical certificate or certificate of capacity
- Email or post letters regarding – Travel, insurance, GP, rehabilitation or external referrals.
FOR A REVIEW APPOINTMENT
For your review appointment please bring:
- Any new scans or test results
- Any medications that you have been taking.
